Medical homes have historically been tested in primary care, but they have been fundamental as a response to the former MACRA legislation, now called the Quality Payment Program. These are two real life examples of successful medical homes in the treatment of IBD and Cirrhosis. These medical homes demonstrate successful coordinated care across a spectrum of providers with a team approach and an improvement on patient care through accessibility and collaboration.
